Your First Visit

Before you arrive

  • Complete your online forms if available
  • Bring your photo ID and insurance information
  • Bring the referral or prescription if one was provided
  • Bring relevant operative reports, imaging reports, or surgeon protocols when available
  • Wear clothing that allows the therapist to examine the involved area

During the evaluation

Your therapist will review your history, symptoms, goals, precautions, and relevant medical information, then assess movement and function. The evaluation is used to establish the treatment plan and initial home recommendations.

After the visit

You should leave with a clear understanding of the plan, the next appointment, and what to work on between visits.
